Based on the DSM-5 Diagnostic Manual, you can easily see any patterns that may emerge in your child’s behaviors. The screening items cover categoies of Inattention, Hyperactivity, and Impulsivity.
This screening checklist is NOT intended to be a diagnostic tool, nor is it an official reporting form for medical or psychological testing.
