This special needs IEPs teacher form satisfies the need to reduce teacher time and to provide required data of present levels of performance!
 
I developed this form to elicit the needed information from Gen Ed teachers and other school professionals for the Present Levels of Academic Achievement and Functional Performance Or Present Levels of Performance section of the special needs IEP. The form covers the skills typically included in academic skills included in a student’s IEP, but saves teachers time in filling out the needed current levels of performance by presenting a checklist type of form.
 
I created this form because classroom teachers would often give anecdotal information without instructional grade levels or specific data records of student scores. Teachers were willing to have input and spent time providing information that didn’t always substantiate need for special education. This teacher form satisfies the need to reduce teacher time and to provide required data of present levels of performance.
 
This form is in pdf file format. The purchasing teacher may request a file created in MS Publisher so that electronic files can be exchanged with teachers. Email is provided in the directions within the file.
 
The purchasing teacher may provide copies to teachers for the students on their own case load. If a school or district wishes to use this form for multiple case managers, a license should be purchased for each Special Education case manager developing special needs IEPs.
